SSI - Servite School Inc. Enrollment Form

1. Prepare all the requirements and secure an application form through our website.
2. Submit the requirements (scanned copy) and fill out the enrollment form completely. Please refer to the list below.

 Note: Hard copies of F138 & F137 will still be given to the Registrar's Office.

1. Photocopy of PSA Birth Certificate
2. Baptismal certificate (2 photocopies)
3. Two (2) recent 2x2 ID picture
4. Original copy of Good Moral Character
5. Original and Photocopy of Report Card
6. The school will then evaluate your application. You will be informed to pay fees through the School Cashier or G-Cash/ Bank Transfer and upload a copy of your proof of payment through our Facebook Page at Servite School.
7. Official Receipt will be given after successful enrollment.

Note:

1. All incoming Grade 7 students are required to fill out the ESC enrollment/ contract form.
2. For Students with Old Balances: Pay your Account Balance via BPI/Gcash (send a proof of payment to our Facebook Page @ Servite School).
